Position description: Adult Day Services Program Coordinator

The mission of Neighborhood House is to enrich, empower and educate children and adults through quality, affordable day care and support services. Our Adult Day Services (ADS) program serves aging and disabled adults with a therapeutic recreational day program. The Program Coordinator is responsible to implement programming to meet diverse ability levels. The successful candidate will maintain current Utah license and certification, manage all recreation therapy for the site, including development of a quality assurance process, intake assessments with new clients and caregivers, development of client care goals, tracking and updating progress reports, and supporting communication between clients, caregivers, and ADS. Duties include managing program aides, assisting in budgeting program activities, overseeing transportation schedules, and facilitating resolution of staff concerns, in conjunction with Program Director.
Responsible for the program’s day to day functioning and the comprehensive care and well-being of our program participants.
Implement a high and low functioning program for approximately 60 clients
Create and monitor individualized care plans for each client
Create and track program outputs, outcomes and measurables
Provide daily written schedule for aides to carry out calendar activities
Complete and distribute monthly activity calendar by the 25th of every month
Be a resource for program aides and provide support as needed
Manage inventory and purchases for program supplies, equipment and activities
Plan, organize, and implement client therapy and client/caregiver events
Meet with health provider agencies as required for individual clients
Write and distribute a monthly newsletter
Develop quarterly goals for the program with the ADS Director
Required Qualifications and Skills, Knowledge, and Abilities
Must hold a current license in the State of Utah as Certified Therapeutic Recreation Specialist (CTRS), Physical Therapist or Registered Nurse
A current, valid State of Utah Driver’s License
Clean Department of Motor Vehicles record and signed form giving NHH permission to run a motor vehicle report
Ability to train and drive a 12-15 passenger van
Demonstrated ability to physically assist clients with ambulating, toileting, dressing, lifting 50 lbs for more, feeding and other needs as assigned
Communicate effectively in English, proficiency in Spanish desired
Pass a BCI (Background Screening) & Drug Free
Demonstrated desire to care for people, including the frail elderly and/or other adults who are unable to live independently.
Solutions oriented. Able to problem solve and develop answers with minimal
direction, capitalizing on opportunities to learn and add value.
Highly computer literate with a proficiency of Microsoft Office, and client
tracking software database(s).
